I mite be wrong but Provoke runs on a 30 second timer within itself like

when u provoke, you create a certain amount of hate (a rather large one) onto the target ... but that hate diminishes over the 30 seconds.. so if u dont do anything to it for the 30 seconds... u will lose the hate u had on it

for the sleep issue, i believe that the sleep hate was still there even after the provoke hate diminished so when the monster woke up it goes after the person which had the most hate on it... aka the sleep caster

Hate dimishes when the person who creates hate is hit by the mob.

Thus after 4-5 hits the provoker had more hate than the sleeper.
