Do they take Visa cash card? It's a card that takes a small amount of money and is rather versatile as it's easy to manage...

Does it have to be under my name or i can use my parents' name as well?

You can use any card as long as you have the owners permission. I use a visa card, as long as their is money (or credit) on the card then each month it will be accepted, should no money or credit be left on eh card then your account is frozen and you can't log in until funds are paid to SE.

You get charge at the begining of each month I don't know how much it costs in the US but I pay about £9.69 a month (including an extra content ID) thats about $19 (US) a month. You have to register a credit card even though your first month is free, however you must cancel you account if you do not wish to play past the first month or you will be charged, they normally charge on the 2nd of every month.

You can get prepaid credit cards now

One that comes to mind is the Virgin one. There is no way possible to get into any debt because of them as you can only spend what money you put onto it. The virgin one I know you can also buy people as a sort of "gift card" It's what I bought my little sister instead of buying her something random I charge her card and she goes off shopping.

They are very cheap and usually only have a small service charge. I think they seriously are great ideas for kids that aren't able to use their parents credit cards.
