Help. Not letting me reactivate account.

I have no idea why I am getting this screen when I try reactivating my content ID. To make sure I still had my characters I almost went through with the process last night, but I decided to wait till I got home from class this morning. Can anyone with the JP client help translate this screen for me? :p

just to clarify, 1/4/2006 is April 1st for those of us who are used to US date notation.
