Did you keep a copy of your POL ID and Password? That really all you need to access your account after you re-install the POL and FFXI software. If you have not kept this information, then the best you can do is contact Square-Enix, though I dont have any information on how to do this unless you speak Japanese.

Member Password: Optional, I dont believe you need one unless you really want one. If you do type in a password, then be sure to write it down. This one you make up as you wish.

PlayOnline ID: The POL ID provided to you during registration

PlayOnline Password: The POL Password provided to you during registration. Now with the password, I dont believe you have to input it here, but then it will ask for it every time you try to log in. Where the double blue arrows are, you need to click there... a pull down menu will appear and you need to select the star option. I think its a star, Anyway, you need to select whichever option will make the text boxes active. This means your going to have POL save your POL password. Type in the password and then select OK.
