I've only got it when I've turned off the xbox without shutting down the POL viewer thus corrupting the installation, in his case it will do it every time without fail when he starts the game, occasionally prob means youve played the crap outa the disk like i did and its scratched )

Is it the vanadiel 08 disc? If so, try installing the disc to the hdd via the nxe. I had a cannot read disc error after the game froze up during campaign and I had to restart my 360.
