1) RTFM

2) Did you even log into POL and click on all the options available? If you would have, there is one called Content ID and when you click it the next options will have one that says Cancel Content ID.

3) And if you can't log in because you are like the OP and have forgotten/lost the information you need to log in, then:

The web chat is the better option, better results and not hanging on a phone for ages costing you $. if you still have your credit card details, you can get your username and password from them, they will ask for your game code (on the booklet), name, address you registered with and card details you paid with. They will tell you your username and give a new password so you can log in and cancel your ID using the methods above.

Obviously you guys have not tried to cancel your account before. Most of the time when you try to cancel your account through the game it just reads and error and does not work. Then when I finally did get it to work and it said my account was canceled it charged me another 6 month period later on. Then I got to play the fun hassle of calling up their help center which is extremely unhelpful and it is very hard to cancel through the phone service. Their subscription cancellation service is an absolute nightmare. I am still trying to get them to cancel $85 in charges (not sure why I dont remember 6 months costing that much) 4 months after I quit and uninstalled this game and received overdraft charges because I wasn't even supposed to be paying square enix.

I have played at least 8 other paid in the box MMOs in the past and every single one I have tried except FF11 has been extremely easy to cancel. You just tell them you want out and optionally provide a reason why and they say sorry to see you go done.
