Actually... I read in another post you need to put a kanji name in the first line and katakana name in the second, if this is true the below should work for you...

kanji:
宮原


katakana:
ミヤハラ
アユミ

the format above is:
lastname
firstname
(which is how i read they should be input - lastname first, firstname second)

the name is AYUMI MIYAHARA
